Why BBL was dangerous — and why that changed
The old core problem was the depth of injection. Injecting fat into the muscle (intramuscular) exposes the fat to a large vein — and fat entering a vein can cause a fatal fat embolism.
In 2018, the major international plastic surgery societies issued clear guidance: subcutaneous injection only — never into the muscle. This single change reduced death rates dramatically.
Before 2018
A historical death rate that reached 1 in every 3,000 procedures — the highest in cosmetic surgery. The cause: injecting fat into the muscle in dangerous venous areas.
After 2018
With adherence to the correct protocol (subcutaneous injection only + ultrasound imaging during the procedure), the rate of major complications dropped very significantly.
How we perform BBL safely at our center
Subcutaneous injection exclusively
Never any injection into the muscle — this is a fixed rule with no exceptions. Fat is injected only into the subcutaneous layer.
A sensible injection volume
More injected fat does not mean a better result — it means greater risk. We determine the volume based on what the tissues can safely accommodate, not on the patient's wish alone.
A specialist anaesthesiologist
Blood pressure and oxygen are monitored throughout the procedure. The patient's position on the table is monitored to prevent compression of the large veins.
Sitting instructions
No direct sitting on the buttocks for 6 weeks — this is not a myth; it is essential for the fat to survive and the vessels not to be compressed. We give you a special BBL pillow.
The questions you must ask before a BBL at any center
⚠️ Ask your center these questions directly:
- Is the injection subcutaneous only, or into the muscle? (The correct answer: subcutaneous only)
- Do they use ultrasound to guide the injection?
- Who is the anaesthesiologist who will be present throughout the procedure?
- How many BBLs has the surgeon performed? And what was the last serious complication they faced?
Realistic expectations from a BBL
- BBL does not mean huge buttocks: BBL improves shape and projection and adds a waistline — it is not for someone who wants exaggerated enlargement
- 60-70% of the fat survives: the rest is absorbed — the surgeon injects slightly more to compensate
- It is affected by weight change: if you lose or gain a lot, the buttocks will be affected like any fatty area
- Reference photos: set your expectations with photos of a body that resembles yours — not a celebrity with a completely different build
